The picture is a severe case. Most of the time, its simply wrinkles that you get from staying in the bath to long. This is cured by letting it dry in open air.

You wouldn't have to go to a doctor unless the problem is severe enough to warrant it. like the picture originally posted, This can lead to infections or more series problems if the case is severe enough or treated improperly. If you are ever unsure if you should see a doctor, you should go just in case.

EDIT: Forgot to mention. Sometimes when the skin becomes over hydrated it dies , which is most likely what you see in the picture. For something like that on a large scale, see a doctor.

Wrong, it can and does fall off from being over hydrated (hyper hydration). This is called Skin Maceration.

How to treat maceration if you have it

There are other sources, but my history is starting to look strange with all the pictures, you can do more research yourself. But I'm not saying this image is defiantly skin maceration, but it can and does happen. From working as a cook for long periods of time, I have experienced similar things on a smaller scale from the sweat, caused by an allergy, under my work gloves. If agitated the skin would fall off.

EDIT: For those who don't know about skin layers. Wiki explains is well enough. The germinative layer is the bottom layer of the epidermis skin layer.
