You do not neet to connect to iTunes to approve new SIM. You DO need to connect it to iTunes once and confirm the unlock, can be done with any SIM.

Is usually in instructions given by your service provider as soon as they have unlocked. To connect to iTunes and check for update. The iTunes will confirm that your phone is now unlocked. Just reply to them nicely. Stating what all fixes you have done. Also whatever they recommended in that email.

They can either inform you of another new fix that you may have not known or they will refund.

They will cover all their bases before they refund. No company ever refunds instantly on an email/support ticket. This way I could download and play any game I want. Then drop an email and get a refund.

If the game is not working, chances are most of the people will just complain through official support and ask for refund. They do not know the diff b/w people who have attempted the update fix and those who did not. Standard procedure to inform/ask them to try this new fix. If it is still not working, refund.
