I'm no good with Photoshop, but I can tell you about the last full arrest I ran as an EMT. In short, the woman died because she was too fat. I don't mean that's why she went into full arrest-- I mean she ultimately died because we were unable to give her the treatment she needed due to her weight. We couldn't intubate her because no one could put enough force behind the laryngoscope to lift up the bulk of her lower jaw and throat. We couldn't get an IV because there was too much fatty tissue between her skin and her veins, so we couldn't give her drugs.

The family sat in the next room in shock while we flailed on this poor woman for about two hours, waiting for enough personnel to respond so we had the manpower to get her into the ambulance. It then took us another 30 minutes to move her outside. At the hospital, a mini-rescue from the city fire department had to meet us there so we had the manpower to pull her out. The docs met us in the ambulance bay; they didn't even bother waiting until we transfered her to a hospital gurney before calling out the time of death.

At the autopsy, it was discovered that she died of a simple pneumothorax, something that is very easy to treat. If she had weighed even 250 pounds, we could have saved her.

This comment is unrelated to the thread. I just want to thank you and all the other EMTs for what you do. An EMT saved my sister's life a few years ago when she OD'd. I consider myself pretty tough spirited, but I couldn't do what you guys do day after day.

One of our family friends told us a story about this morbidly obese (like 500 lbs) guy who needed an MRI or something, but wouldn't fit in the machine. They tried everything. He was too fat for an ultrasound too. Finally, she ended up calling the local Zoo to ask to use their elephant MRI machine. They laughed, told her they got prank calls like that at least once a day, and to not call again. The guy is still undiagnosed.

EMTs/Paramedics are not thanked nor paid enough for the work they do. PTSD pretty much comes with the job. Thank you for what you do. I'm looking at getting my EMT cert, but I am simply cannot stomach the idea of making it my job.

You can get your EMT cert and volunteer. That's what I do. You'd be amazed how much you can "stomach." When your training kicks in, all you're focused on is the next thing you need to do to get the patient to definitive care. I cannot recommend volunteering as an EMT enough.

::Please don't read this as criticism, but as curiosity in extreme cases::

Wouldn't the pneumothorax be relieved by "stabbing" a trochar into the chest along the side of the ribcage? Even if you couldn't get intubated or an IV, would an opening into the thoracic cavity take her from needing critical care to needing supportive care (or at least buy some time before she was critical again)?

In the field that day, we had no way of diagnosing her pneumothorax. We got called for a woman in full arrest. When we got there, our initial concern was restoring her heartbeat and respirations. We never got past that point. If we had been able to establish a definitive airway (getting a tube into her lungs) we might have been able to listen to lung sounds and realize that she had a pneumo, but we were stuck using a bag-valve mask and could barely get air into her lungs. The call was a clusterfuck from beginning to end because every effort we made was frustrated due to her sheer bulk, but legal ramifications (and an asshole of a Med Control doc) forced us to continue.

TL;DR Ideally, yes-- but this call was as far from ideal as one could get.

This is my personal timeline, and I did it all through exercise and healthy eating. My diet isn't complicated, just eat less sugar whenever possible, and eat more whole grains and vegetables. Frozen vegetables contain more vitamins than fresh, and come pre-cut so they're easily added to a dish. Peanut butter is a life saver! It tastes like it's bad for you, but doesn't contain much sugar. Exercise is whatever I want, as long as I do it 3-5 days a week for a least half n hour.

Just wanted to say that you look amazing!! When I graduated from high school in 2003, I weighed about 200# and was a size 16. By 2005, I weighed 126 and was a size 4. Then I got a job at a restaurant and got engaged... now I'm 145 and a size 8. It's sad what access to unlimited junk food and a man who loves you no matter can do to your waistline. Haha. I need to back in that healthy mind frame. You are an inspiration!!

Don't think of what you're doing as losing 100 pounds. Think of it as developing a way of eating/exercising that you're going to be able to sustain permanently. The weight loss is a pleasant outcome of this new way of life, but not the only purpose for doing it.

If you think you'll lose the weight and then go back to how you were eating before, it will never work.

Here's a fucking motivational poster. That's my insulin and syringes that keep me alive. I have type 1 diabetes that I got at age 16 from no fault of my own. By being overweight you are putting yourself at risk to get type 2 diabetes. Trust me, you don't fucking want diabetes. It really pisses me off to see people throwing their lives away because they wont get off the god damn couch and exercise and eat a little healthier. I would do just about ANYTHING to not have diabetes, but I didn't get that choice. You have that choice. Don't be a dumb ass.

Seriously, I hear ya, I had a double lung transplant a few years ago because i was born with cystic fibrosis , and everytime i see someone smoking I rage. Hard. I wish there was a way to get them to see how it felt to have 17% lung capacity for even an hour.

On a side note, I know have cystic fibrosis related diabetes which is exasterbated from my anti rejection drugs. Doing insulin 6-10 times a day is no fun either. But a few needle pokes is a small trade to be able to breathe. I find myself specifically avoiding sugar unless I'm going to be eating anyways. Injecting myself just to drink a soda isn't worth it.

I'm sorry about the cystic fibrosis, but I would like to clarify further for those without diabetes that its not as simple as a few needle pokes. If I could just take insulin and be ok for the day, that would be awesome. I don't mind the needle pricks at all. Its the ups and downs all day long, planning your day to make sure there is always food close by, waking up at 3am when your blood sugar bottoms out and you almost pass out, feeling so tired and generally shitty that you just want to sleep all day, that stuff is what I hate the most.

Well, here's some pseudo-facty stuff: Your actual weight is really just the integral of your eating habits over time. When you improve your eating habits, you get a number of the benefits in fairly short order, even if it takes a while to drop the excess weight. There have been studies where they watched blood markers and observed this. I personally managed to drop my cholesterol level from awful to excellent over a period of maybe six month by eating an extremely healthy diet, even though I barely made it out of the obese range on that diet.

The hardest part, of course, is keeping it off once you've lost it. So far I've always failed, so I don't have many ideas on that.

Here is the stuff I learned on my 70 lb journey over the last year:

  • Working out is critical to mental and physical fitness. You will be happier and balanced if you work out 5-6 times a week. 30 minutes is adequate, 60 is perfect. Remember that is less than 1/24 of your life.

  • Weight loss is all about what you eat. An hour workout only burns about 500-600 calories and you can eat that in 10 minutes at McDonalds.

  • You need to learn your metabolic rate and set a daily caloric goal. For me to loose 2 lbs per week, I ate only 1500 cal per day for 4 months.

  • A net deficit of 3500 calories = one lb of weight loss. So 500 cal deficit a day will make you loose 1 lb per week. 1000 cal per day = 2 lbs per week. You should not try to go faster than 2 lbs per week.

  • NEVER let yourself get hungry. That is when the body starts storing fat and cannibalizes muscle.

  • Eat 20-30 g of protein after your workout to trigger protein synthesis and speed recovery so are not sore the next day. I like GNC AMP. I also do egg white omelets all the time.

  • Fat does not make you fat. Calories do. Just watch your calories.

  • Carbs are just energy and very little gets built from them. Your body will change when you get protein and vitamins and minerals in there.

  • Find a workout buddy. It is so much more fun with a friend.

  • Variety is key. Make sure you are doing many different kinds of workouts. Weights, Running, Yoga, Intervals, Pull Ups, Push Ups.

  • Learn exercise moves that use your body before going to machines.

  • Write down everything you eat. This drives more discipline than anything else. I have tracked all of my workouts and intake for 324 days on my blog

  • Make sure you have these kind of foods readily available as snacks: popcorn, protein bars, almonds, pistachios, apples, celery & peanut butter, Go Lean.

  • Try to eat 5-6 300 calorie meals a day instead of 3 big ones.

  • Finally, learn to cook. Get some healthy cook books that have calories in them. When you cook, make 2X what you need so you have healthy left overs. Attack cooking with enthusiasm and make it a challenge to make delicious, yet healthy meals.

This is working for me.

I took a picture of myself in the most unflattering way possible. I stuck it up in my closet where I see it every day. Under it is a note that used to say "Why let yourself look this way?" I changed it recently to "I will not let myself live this way"

First off, congrats on quitting smoking. Here's some motivation for you.

My husband (we were separated but still married) just died 11 days ago at the age of 45 from a heart attack.

He smoked and he was about 75 lb overweight. I found him sitting in his computer chair, still luke-warm but too far gone for CPR. He had a couple of server windows open, his email, and a Reddit window. His name on here was gregmess (hence my user name). He didn't post much but he and I read Reddit every day.

My life is a fucking mess right now.

Don't do that to the people you love, please.

FORTY FIVE YEARS OLD. We have a 14 year old daughter. Think about what he missed out on - her h.s. graduation, college graduation, wedding, grandkids....

45 years old.

I think what we're all missing is an education in the correct use of food. Our ideas today are shaped by the food industry, not by what our bodies need. Some people have just been educated correctly by their parents and others haven't. All those "it's in my genes" people and those "I can eat whatever I like" people are just liars. It simply works or doesn't work but they don't understand the basic maths here. While there are some mitigating reasons, most of them have little effect on the core calorific need of your body and the calorific inputs and outputs that you control.

What we really need is better food and diet education.

'Eat less, move more' is a great summary but this isn't a diet, it's a lifestyle change. If not, you'll just become a yo-yo dieter.

Every pound of fat on your body is around 3,500 calories. A healthy man with a moderate amount of exercise consumes 2,500 calories per day; or 17,500 per week. A fat person consumes more because your body works harder. A more sedentary person consumes less. Therefore, if your body needs 2,000 calories and you over feed it by 150 calories a day, you'll be 16 pounds heavier by the end of the year. Kill the 150 extra calories and you won't put on any more weight -- but you won't lose it either. So a diet is Phase I - it's an over-correction until you reach your healthy weight and then you move to Phase II - keeping it the same.

As you age, your calorific needs go down. As you become lazier, your needs go down.

So if your basic need is 3,000 calories per day and you only eat 1,200 calories, you'll lose 1,800 calories per day, i.e. 3 to 4 pounds per week. As you get thinner your rate of loss slows down though because your requirement declines. If you try and exercise to lose 1,800 calories per day you'll need to strap yourself to a treadmill for two hours. So I think dieting is about 80/20 in favour of food/exercise. There are benefits from exercise but there are pitfalls too - i.e. you eat what you've burned off and you over-exaggerate the calories or rely on what the machine says.

It's complex and I've been self-experimenting for 25 years but I say this after losing 60 pounds and with a BMI of around 26.5. Been there, done that and have the M, L, XL and XXL T-Shirts to prove it!

I am not talented, but whenever my husband is having a "Hey, I lost five pounds, maybe my metabolism has miraculously sped up and I can get away with eating this pie/not exercising for a month/drinking sweet tea again, he calls me and has me send him a disapproving look with the caption "YOU FAT". He did the same for me when I was shedding sixty pounds of baby weight.

I don't know if you will read this comment, but I lost 50 pounds some time ago. The best way is to develop a control methodology: keep a spreadsheet where you write down everything you eat and exercise, your weight, and so. It will be much easier to keep track of the progress and correlate specific foods with weight gain/loss.

I am more impressed by people that lose a ton of weight than those that kept it off their whole life.

Be hard core. If there is some super cute, petite girl you have a crush on, look at a picture of her and then set your alarm clock for 6 am. Wake up and go jogging for 30 minutes.

This has to be quick, but three things you won't find in the fitness books.

  1. The calories needed to maintain someone at 150 pounds and 200 are different. What often happens at a plateau is that you're eating at the maintenance level for your weight.

  2. Similarly, once people get to their target weight, they start eating more, thinking that they just need to maintain. However, they start eating at a maintenance level 20 pounds heavier than their target weight. Don't fall into this trap.

  3. İt isn't the 30 minutes on the treadmill that makes as much change as starting to walk the mile or two to the supermarket and taking the stairs everywhere you go. The cardio is about increasing your potential activity level.

There are quite of good stuff in /r/Fitness/. You got motivation, testimonies, and also some tips and stuff.

I find reading testimonies much more motivational than a poster.

By the way, the effects of obesity are so many that I'd find it hard making it fit in a poster, and even then, when you lose much weight you just feel better, it's not something easy to compare. Read some stuff in the fitness subreddit, it's really good.

Best wishes to you, OP. Since April 25th (when I started keeping track of my weight loss) I've lost 32 lbs - from 205 to 173. I'm trying to lose another 10 or so, should take me another 3 or 4 weeks. I'm a 6'0 guy and I think 165 is right around where I should be...

It's such a great feeling to shed a lot of weight. I've been chubby/fat ever since I graduated high school, which was 9 years ago... but back in January when I noticed my weight hovering around the 215 lbs range, I decided I just couldn't be like that anymore. Best decision I ever made!

You know what's the best motivator? Facts

Despite popular belief, there is no such thing as "easy pounds". Trust me. If you eat less calories then you spend, you will lose weight, no matter what, and that rate loss does not correlate with how much you have already lost. What I mean is, if you need 2000 kcal to your daily functions, and everyday you eat, let's say, 1800, eventually, you'll lose weight, no matter how much weight you have already lost.

So why there is this popular belief about easy pounds? It's because people usually go on very hard and unrealistic diets, starving themselves for short periods of time and losing lots of weight early on, then, they just can't keep up with it, and start cheating on their diets, and the weight loss stop. You now this, you probably been there. So, there is no thing as easy pounds, less calories then are being burned it's what this is all about.

Other facts: You need to burn 3500 kcal to lose one pound of fat. So 350 less calories for 10 days, and there you go, one pound lost every 10 days.

So keep you goals realistic, and the rate of weight loss low. What's important here is that you know that every pound lost is a pound you'll never see again in your body.

But aren't the basic calorie requirements more for a large body than a small one? I thought that was why the first part of losing weight was easier than the last, because you go from burning 2600 Cals a day through body functions to burning only 2000 and your diet needs to be changed even more.

I use the 'my fitness pal' app for my iphone, it's free and it's got a massive library of foods. I stick to a strict amount of 1350 calories a day and do yoga and pilates and I've lost 21lbs in 2 and 1/2 months. You can definitely do this, it's just food, it's not your friend and never will be (I've only just realised this.) When you're craving a snack, have something low cal, or brush your teeth, or do a 10minute workout. Eat small amounts throughout the day, I know the craving to want to be really full but you won't crave as much if you don't have that full, hungry, full, hungry pattern. Good luck <3

I dunno...looking at pictures of fat people (or watching those weight loss shows on tv) almost always have the REVERSE effect on me. I guess I think, "hey, i might have put on a few pounds in freshman year, but at least i'm not overweight, and i've got a longgggggg way to go before i'm as fat as that chick. I can afford to eat that cookie!"

and then looking at pictures of super skinny hot chicks doesn't do it for me either because i'm like, "my thighs will never be that slim, regardless of how much exercise i do. even when i was UNDER weight i had proportionally thicker thighs than that chick".

so nothing really does it for me.

the best way for me to lose weight is to do it without thinking i'm about to do it. E.g. go on a 2 hour long bike ride for fun with a friend and race him (i'm super competitive), or go dancing, or go swimming. i can never motivate myself to just 'hit the gym' (even having paid hundreds of dollars in athletic fees at my school doesn't motivate me) or just 'exercise'---it kinda just has to happen without me realizing it.

with that said, the worst demotivator is seeing my teenage brother wolf down a pizza and still end up a tall stick while i eat my healthy multigrain pasta and salad without dressing and somehow still end up rounder than he is. F (my) metabolism.

Working on some weight loss myself, went from 238 to 190 in the last 6 months, while building a stupid large amount of muscle (apparently it's easy for me.) My motivation is my best friend, who went from 300 to 165 in about a year and a half, with major muscle growth, and then placed 11th at our regional bodybuilding competition.

Keep it up, never give up, never back down. Also, motivational films are my jump-off, Never Back Down being one of them. Congrats, and remember to kick ass and chew bubble gum, even if you're all out of gum.

We're not meant to sit stagnant in this life. Move or die. These are the rules. I'm a skinny guy who smokes, so I got changes to make myself. Cigarettes make it too easy for me, to just not eat. I'm at the opposite end of the spectrum from you, but no healthier. I'm starting to eat better, learning to cook a little, attempting to incorporate exercise into my life, and dammit, I will get these fucking cigarettes outta my hand. Why? Like I have a choice? Quality of life, that's why. I keep living the way that I do, and I'm getting kicked off planet earth alot earlier. I wouldn't mind not feeling like a milk truck hit me, everytime I wake up either. I have been swimming all my life and coaching the sport the last 5 years. I worked with a woman who was severely overweight. Within three months she lost 34 pounds. Make sure you add this into your weight loss regiment. Just another suggestion...I loved that same smoking image. I have shown it to a few friends...one stopped...the others are trying to still.
